Access to records

You can request your personal information by completing the Access to Records Request form and returning it to us with copies of 2 pieces of ID (one of which is to be government issued photo ID).

We require signed, written consent from all other individuals in order to release their personal information to you.  If signed, written consent is not received for all individuals, the personal information of those individuals who have not provided consent will be removed from your disclosure. A parent with custody of a child under the age of 16 may request personal information of their child, subject to some exceptions. If you are a person with custody and wish to request personal information of your child(ren) who are under the age of 16, please provide the name(s) date(s) of birth of your child(ren) and confirm that you are a custodial parent/guardian. In some circumstances, we may ask you to provide documentation confirming your authority to make the request in place of your child.

Adoption disclosure

Through Our Agency: The Catholic Children’s Aid Society of Toronto (CCAS) will provide non-identifying information from our adoption related records to the following persons who qualify:

  • An adopted person who is over the age of 18 (if younger than 18, consent is required from the adoptive parents)
  • Adult children of a deceased adopted person (must provide proof of death for the adoptee)
  • Birth family members, including the adopted person’s birth parents, siblings of birth parents, birth grandparents to the adoptee, and birth siblings to the adoptee. Adoptees may also receive non-identifying information about a birth sibling who was also adopted.
  • Adoptive parents

You can request your Non-Identifying Adoption Information by completing the Request for Non-Identifying Information form and returning it to us with copies of 2 pieces of ID (one of which is to be government issued photo ID).

Through Service Ontario: If your adoption was finalized in Ontario, adopted adults and birth parents can apply for additional adoption disclosure information and services information at Service Ontario or call Service Ontario toll free at 1-800-461-2156.

Child welfare record check for employment

CCAS programs and services are designed to meet the needs of the Catholic families and children living in Toronto. If you are a part of the community that we serve and if your employer requires of you to submit a Child Welfare Record Check as a condition of an employment, please download the Child Welfare Record Check Consent Form: Employee Record Check here and fill it out with required information. You also have to provide copies of 2 pieces of identification, one of which should be a photo ID. You can email the form to disclosure@torontoccas.org or by mail it to:
